Community Support

We provide one to one person-centred support in the Community to Young people and Adults. Support is provided by trained Support Workers and Registered Mental Health Nurses.

Our staff are experienced and trained to work with young people and adults with complex needs such as mental health and physical disabilities.

 

We support young people and adults to

  • Access Activities in the Community

  • Attend Court and YOT appointments

  • Attend Medical appointments

  • Access Education, Training or Employment

  • Attend Contact/Family meetings

  • Gain Independent Living Skills such as budgeting, cooking, cleaning and shopping. 

 

Our team of specialised Support Workers also provide Crisis Support, Welfare Checks and a Sitting service.
